I assume it's like the kids who walk around with one pant leg rolled up. Left leg indicates that they're a top, right leg indicates that they're a bottom.
I've often speculated about the origin of the LOLWUT pear. What was the context? What was the author's frame of mind? Unfortunately, that image has been around so long and has been re-hosted so many times that tracking down the original seems like it would be impossible.

On the plus side, that link to the creator's DeviantArt page turns up a lot of interesting stuff. One, in particular, seems especially apropos for threads of this nature:
