why don't manifold makers use studs like this?

Because the pipes coming into the collector normally do not allow room to slip a bolt into the hole on the manifold side and there is barely enough room for a nut and a wrench on the turbo side

I've never had my manifold to head or manifold to turbo studs come loose. The down pipe ones are another story.
