Ok, I've been troubleshooting this for awhile now, and I'm no further to an answer. Injector one shows no response during "injector test" and while driving.
I've tried:
-different injectors (factory MSM and DW 800's)
-different injector locations (swaping 1-4 etc)
-Travis took a look at my ECU
-Voltage confirmed the same as the other rails both by ECU data and multimeter (~11.4V) during "injector test" and simple key in the power on
-different firmwares (V1,V3,V5)
-different maps

The attached injector test is a MSPaint hand drawing of what it's doing. It's not a screen shot. This problem started with the factor injectors. I thought I cooked a factory injector, so I order some new ones. The 800's did the same thing. Sent Travis the ECU to take a look at the ECU. Got it back, and it did the same thing at first, but then all of sudden I got a response similar to the other injectors (grey), but it was never a consistent slope. I tried wiggling the wires to the injectors (all 4) but nothing seemed to be the cause, so I decided to put the car back together. When I got the car back together, it was back to the example picture. Same 3-cylinder Piece of Shit.

Does anyone know any reason why this could be happening?! I haven't driven the car in month. This shit is getting old.:vash::vash:

When I first installed my injectors, I was getting the same type of response. It turns out I accidentally bent the electrical connector on the number 1 injector. It sounds like you might have a different problem though.

It might be worth looking at the adapter board and making sure the solder connection is good.

Did you connect a multimeter to the injector harness clips and then fire the 'injector test' and then see the voltage register?

You could also check the wiring resistance from the adapter board to each injector clip to make they're all about the same. I believe Travis has a wiring diagram for the adapter boards.

I did. The voltages list are a little low due to a low battery but I hooked up a multimeter to a good and bad injector locations. Voltages were the same during accessory mode as well as when I fired the injector test.
I put the factory msm injectors and stock ecu back in and it runs on all 4 cylinders. Travis told me he'd take car of me so that's where we are at right now. The only variable left is the ecu.
