Why is it that dentists, who work on some of the smallest and most delicate parts of the body, are the only medical professionals who still use a syringe that looks like the sort of thing you'd use to euthanize an elephant in the 19th century?

I certainly hope so.

And it appears that the syringe in question actually uses a replaceable, disposable plastic cartridge inside the metal housing, rather than a glass tube as in ye olden days.

It's mostly a visual thing. I hate looking at that damn instrument.

my dentist uses lasers instead of drills. it's the most amazing thing ever, doesnt require general in order to use it.

General?

Most dentists use local anesthetic, if that's what you meant. General is when they put you under completely (eg: for major surgery.)

For a root canal, the drilling isn't the unpleasant part. It's when they start ramming tiny rotary files of increasing length and diameter all the way through the entire tooth and out the root shaft into the inflamed, abscessed bone of your jaw. They're called endodontic reamers.

This is the basic procedure:




Trust me, you need anesthesia for that. A lot of it.


In my case, there was no external fissure, the damn tooth just kinda died. Thus, the hole that had to be made was relatively small, and so no need for a post and crown. Just a regular filling to close the tooth after sealing.

Yah, glass is what all the local anesthetic cartridges come in. I've pushed so hard on it during a PDL injection that it actually shattered.

There are different types of injections. The blue needle tips are the thinner gauge ones that are typically used for anterior to premolar injections. The yellow ones are long and thick for inferior alveolar or Gow Gates injections (bottom teeth) that go almost all the way back to your condyle; thicker gauge because if it breaks, it can't be removed without surgery.

Incisors don't need crowns after a root canal. All premolars and molars do because they become brittle and will vertical root fracture if not done.

I had a patient in dental school that was certain meditation without local anesthetic would be enough during a root canal procedure. It wasn't. This was in San Francisco of course.

Yeah, I can't see that happening...

I'll admit that I whined like a bitch when that needle went into the abscessed tissue during the first half of the procedure two weeks ago. One of the most painful experiences of my life; I'd put it just below when the nerve block wore off in the middle of the night after the reconstructive surgery on my right hand, after I'd gone to bed thinking "Well, I'm not really in any pain, no sense taking these heavy-duty narcotics that the surgeon prescribed."

Protip: if the surgeon gives you the kind of drugs that street junkies would kill for, take them before going to bed after orthopedic surgery regardless of whether you think you need them or not. If you have six titanium rods protruding out of your body, you need them.
