Use remote access like others have mentioned. Set your home router to allow remote desktop requests (port 3389 by default), enable remote desktop on your home machine, and you're more or less good to go. You could just check your IP once in a while and carry it on a sticky note in your wallet, or use a dynamic dns service like others have noted.

On Topic: I can't go through home router. I have an air card on my laptop at home. I could just swipe it and bring it to work, but I would have to disconnect from the network which would be a pain, plus my wife only works part time and it's really her laptop. (Although, the only reason that I bought it was for tuning my ECU but don't tell her.) My old PC was old, but worked OK for what she needed. She is happy with "her" new laptop.
