Use the "invoice #" from the PDF copy of your order that came in an email from "Consumer Affaris" or in the hard copy that comes bundled with your tires. Mine is 5976xxx.
Today is the last day to purchase for the $100 rebate. Before ordering I did a comparison of total prices between an ebay order and an order off their main website. The base price of the tires is cheaper on their website, you still get free shipping, and when I looked there was a $40 rebate in effect. The net result was that the ebay promo saved exactly $25 versus their website. Assuming you can still use the other $40 rebate, you're not taking a tremendous hit if you don't order today, but more money is always better.

Make sure y'all check the fine print on that pre-paid Visa card. Says it expires in 4 months. I know there are laws that prevent loss of value on gift cards but not sure if that applies to these prepaid debit cards. Spend them soon.
