Default Engine cuts out under acceleration

So my car has developed a issue the past few weeks. Under hard acceleration between 2500-4000 rpm the car will completely lose power and the tach will shoot to zero. The engine stays unresponsive until I lift and then it comes back to life like nothing happened.

Normal city driving going to school and back I never have this issue. It only occurs when trying to accelerate hard. I have replaced spark plugs, plug wires, changed out coil pack with one I know works, cleaned engine grounds... Long story short I don't really know what going on with the car. I had a fellow forum member helping me tune the car but haven't hear back in a while. I've attached my tune along with some logs of this issue happening. Hoping for someone with a bit more knowledge and experience to point me in the right direction.

The car is a 1.6L
MK Turbo setup
diy MS3x
stock fuel pump
seq injection
stock ign system

I think that's your AFR safety system cutting fuel cause you're a few points too far from your target. That's right where AFR safety drops from .4 allowable error to .3, and you're .4-.6 off target.
