Yes it fits a 94+ Head. I bolted it onto a 96 head, it was on a 99 head previously.

Cold side is .60

I can't see any markings on the hot side so I can't say 100% what it is.

390/300wheel setup @ 14psi on a 1.9L 9:1 compression 99 head with a little work.

Intercooler setup is sold. Honestly wouldn't be worth shipping intercooler compared to what you can buy them for (heavy).
