Try disconnecting the injector plugs one at a time and see if there's one you can pull that doesn't affect the way the engine runs. Is there?
What if you put the #4 injector plug on a different injector?
If I do that and the problem is still on the 4th injector what then. I haven't tried it yet but if that were the case. If the problem moves with the injector it's the injector that's bad. If it stays in the same position what might the cause be?
Replaced fuel filter and put in new injectors but I still can't get TS to read the same AFR as my DB Gauge. Changed both outputs in LM Programmer to 0=7.35 and 5=22.39 or what ever is default from factory. Changed WB settings in TS to LC1/LC2 default and tried it as custom linear with no effect. Wideband is grounded at the ECU ground at the front of the block. Gauge ground is tapped into that ground as well. WBO2 sensor is plugged into the first stock O2 sensor located after the header. LC-2 output #2 goes to pin 21 on the MSPNP. Are no other pins needed? I havent added anything else (IAT or EBC any other options).
I don't want to have to undo all my wiring and rewire everything but at this point i don't know what else to do. Ive gone through a ton of different threads about TS not syncing with afr gauges and tried what was suggested but havent found a solution

How far off are the readings - are we talking half a point, or seven?
Which wire do you have connected to the MSPNP? Yellow or brown?
Have you made sure the brown wire is calibrated for wideband output and not simulated narrow band, using the LM Programmer?
The settings sound OK - you don't by any chance still have the stock narrow band hooked up too? Can you check the voltage the wideband is putting out?
I replaced the first O2 sensor after the header and the stock connector it's unplugged. Is that not the right one? I'll have to check the voltage when I get home.
Can you check to be sure they both have the same voltage? If you programmed them to be the same and they're not, you'll want to give Innovate a call.

Measured the two outputs where they go to the gauge and to the MSPNP. completely different numbers. So I got in contact with Innovate. Doing some troubleshooting with them right now. Thanks Matt for your help so far. Checked the readings from logworks and compared them to tunerstudio readings after I swapped the gauge and MSPNP and they are only off by maybe 0.2 or 0.3 so I tweaked the settings a little bit to get it closer together.
So far I've got it to idle pretty well using Autotune. Changed the Req Fuel for my ID 725. Adjusted my Accel enrichment so that it doesn't stutter/bog when I hit the gas. Ran Autotuned while i drove around for a bit and it seems pretty good.
Not sure what to adjust now to get it running even better.
