Found my oxygen sensor problems
Turns out I didn't reassemble my manifold/turbine/DP in the correct order after I finished with the motor mounts and I left a little gap between the manifold and turbine. Oh well, at least it is fixed now.

The center section blocks the two front-most bolts and prevents me from fully tightening them. I'm not sure how much of a leak there is, it's hard to tell from eyeballing it alone, though it does look a bit misaligned or not quite flush. I might have thought it was tight but been mislead because of tension from the weight of the downpipe and lack of access.
I won't be able to tell until I take off the center section and loosen the DP from the exhaust to see if it makes a difference.
Everything about my O2 sensor problems points to an exhaust leak of some sort. I can't feel or see any holes or cracks in the manifold or housing. I'm at a loss for ideas if this doesn't work.

Well I took everything out and inspected it with a fine-tooth comb and everything looks perfect IMO. No holes, cracks or anything. A little bit of soot under the exhaust gasket but that's about it.
If I still have problems after a careful reassembly, I dunno what the cause is.
The ceramic on the manifold is finnaly deteriorating after about 4 years. Everything is in remarkably solid shape after all these years (5+) of 1600+ degree temps.
