Default Spark wet-out symptoms

Simple idiotic question because I can't seem to figure out what is happening with my car. 1.6 BEGI setup, DIYPNP running about 12-13psi out of SR20 T25.

This has happened twice thus far. Once (back at about 9-10 psi, pre clutch), I was flooring it onto an on-ramp in first and heard a gigantic bang followed by a huge cloud of black smoke behind me. Car sounded like it was running on less than four cylinders for a bit. Pulled over, turned the car off, turned it back on and it's fine. A month later, the same thing happens, no bang this time (it was cooler out, so I assume whatever unburned gas didn't ignite in the exhaust) but still a huge cloud of black smoke. The 2nd time was in 2nd gear around max boost at 4-5k rpms. Car sounded funny for 5-10 seconds, let it go back to idle, blipped the throttle a couple times and it came back to normal.

Sound like spark blow/wet-out? I think my plugs are gapped around 0.35 last I remember.

Behind me, pretty sure it was just a ton of fuel lighting off in the exhaust

Do you have a wideband? Can you log data? That is, by far, the best way to diagnose stuff like this.

Failing that, are you able to pull your plugs for an inspection immediately after one of these episodes?

Yeah, I have an AEM Failsafe that logs non-stop, I'll have to check that. Not sure if I can figure out when that run was though. I'm going to pull the plugs now and check the gap.

Edit: I will say, when I was cruising to pull over, my AFRs were what I'd expect at 15.5ish (motor still sounded funny at this point).
