Honestly, the system isn't unsafe or scary at all. It's much cleaner than is typically portrayed in those old pictures and films, graffiti is practically nonexistant, and while there is a faint but persistent aroma of urine throughout most of the system, the fragrance of the rat poison tends to compliment it nicely.

Japan's subway stations weren't bad. I'm not sure why anyone would get lost. The only problem I had was that they have 2 different companies running the lines, so you had to make sure to get the pass that worked for both or know exactly which line you needed.
