During DFCO on over-run, a vacuum can be developed in the exhaust.

Throttle is closed vacuum is generated on intake down stroke. Compression brings the pressure back to around atmospheric. Exhaust valve opens somewhere above BDC basically unleashing a substantial vacuum on the exhaust. The only difference from this and truth is that a small combustion event occurs, but the rag was sucked in on decel, in which if he is in full fuel cut there is no combustion so a substantial vacuum occurs.
