I remember seeing that on the news a couple months ago.. Just kinda makes you go "WTF??"
I'm not a photoshop expert by any stretch, but just because the intake manifold and motor mounts aren't perfect to your standards doesn't mean the engine isn't in reality sitting where its shown.. The only thing that'd make me really say its probably a modified picture is the overall shitty grainy quality of the photo.. Even shitty iphones can take better pictures than that thing.. Plus the exif data is all gone..

For instance, have you ever has Cincinnati-style Chili (Skyline, Gold Star, etc)? It is essentially a watery sauce consisting of finely ground beef, lightly spiced and with hints of chocolate and cinnamon, which is poured over a plate of spaghetti and then topped with entirely more shredded cheddar cheese than is reasonable or prudent. Kidney beans and/or raw chopped onion are optional, and are added as toppings- never mixed in.
It sounds absolutely disgusting. And frankly, it's not much to look at either. But it is gooooooooooood. On one occasion several years ago, I actually altered my flight plans coming home from NY to go through CVG instead of ATL purely on the grounds that there is a Gold Star Chili restaurant in the middle of Concourse B at that airport.

(That's not just marketing. They really do put that much cheese on top of the thing.)
Found two very interesting things on google maps.. Anyone guess what they are? Technically neither are related to another, however their geographical location is very close. I wouldn't be surprised if anyone figured out the first, the second though I'd be VERY impressed..
