A "green card" means that you are a legal permanent resident. You're not a citizen, but you have many of the rights (and obligations) of a citizen. You have to pay taxes and register for the SS, but you also have the right to stay as long as you wish, travel freely in and out of the country, etc. Green card = immigrant.
A Visa is a completely different thing. They are temporary, and are issued for a specific purpose (work visa, student visa, tourist visa). As it says right on the form, if you are here on visa, you are not required to register.

Most of the international students I know that originally planned on going back home have found great job offers and are now going to stay here. Once you acclimate to the different culture it's easy to feel at home.
Once he applies for green card or citizenship he would have to apply again. I'm pretty sure I've sent in multiple Selective Service forms between voting registration, driver's licenses and school. And I'm about to no longer be required to.

The thing about the MSS forms in his case is that he's 24 (pursuing a post-graduate degree) so he'll be 26 by the time he finishes school.
